The State of Colorado has been working with a cross state agency team to identify Coloradans who are in need of an in-home vaccination. Initially, this effort focused on Colorado’s Medicaid population, but is ready to expand this option to non-Medicaid eligible Coloradans. At the bottom you will find a Google Form link that can be used to register individuals in your communities who require an in-home vaccination.

Identifying these individuals is two-pronged approach:

  1. We will be asking trusted community partners, such as the Area Agencies on Aging and Centers for Independent Living, to input individuals they know within their regions who may need an in-home vaccination.
  2. Beginning the week of May 3, the 1-877-COVAXCO call center will be able to register individuals for an in-home vaccination if they are unable to make it to a community-based clinic. A caregiver, family member, friend, or the individual themselves will be able register for an in-home vaccine through the call center. They simply need to call the above number and indicate that they, or a loved one, needs an in-home vaccination and provide the necessary information to the individual.

Once an individual’s information has been input into the system, we will separate entries out by county and share updated lists weekly with either local public health agencies who currently have local solutions in place, or with the Colorado Department of Public Health and Environment (CDPHE) for deployment by their mobile teams. Once a private sector contractor is in place with CDPHE, that group will handle scheduling and in-home administration. The lists provided to vaccine administrators will be bumped up against the Colorado Immunization Information System to ensure individuals have not already received a vaccine.

Individuals will be contacted at least a week in advance to confirm they still need the in-home vaccination, ensure the administrator will be able to enter the residence, and provide a window of time for the appointment. In most instances, a follow-up call will be made 24-48 hours prior to the vaccine appointment as a reminder. It is likely that the vaccine administrator will need to be at the individual’s residence for around 45 minutes. If the administrator arrives and no one is present after 15 minutes, they will have to move on to the next appointment.
